DOT Physical & How To Prepare For Your Appointment

Medical assessments and diagnosis for DOT physical exams help to certify that a driver is capable of operating a commercial motor vehicle by checking for mental, physical and organic conditions. Medical assessments such as vision, hearing, blood pressure/pulse, urine specimen testing and a review of medical history will be conducted by the physician.

All DOT physicals and drug testing are done by Dr. Jessica Hansen-Barnett D.C., FASA who is a Certified Drug Screening Professional and who operates under Federal guidelines for performing physical examinations and DOT drug tests. She received her certification from Northwestern Health Sciences University registered through the National Registry of Certified Medical Examination #9381299648.
DOT exams are not covered by any insurance. We keep our fees lower than most in the area. Complete history, exam, and urinalysis is only $75. Payment is due at time of service.

You'll need about 45 minutes to 1 hour for the exam and paperwork.
Remember to bring the medical records you’ve prepared in advance, including your list of medications.
Remember to bring eye glasses and/or hearing aid if you use them (it’s surprising how many people forget this).

*Diabetic Condition – Bring a list of medications and name of prescribing doctor.
*High Blood Pressure – Bring a list of medications and name of prescribing doctor.
*Heart Condition – If you’ve had a heart attack or cardiovascular disease, you’ll need to bring a copy of your last stress test.
*Sleep Apnea – Bring a copy of your latest sleep test results, if you have one.
*Medications – List all the prescription medicines you take, including strength and dosage. If you regularly take over-the counter medications, such as antacids or allergy pills, list them, too.
*Contact information – Have names and phone numbers of your doctors in case you need to call and have them fax missing information.
